 Patient Responsibility – Keep Your Model!

 

At the time of crown or prosthesis delivery, you will receive a model  (a 3D replica of your teeth).

 

 Please keep this model safe!

To claim a warranty, you must submit this model during your visit.

 Without the model, the lab will not process the warranty.

 Warranty is Valid Only If:

1.  Regular 6-month check-ups at SmileSparks are attended

2.  Good oral hygiene is maintained

3.  No misuse, trauma, or accidental damage

4.  No external lab or clinic modifies the prosthesis

5.  Bruxism or other conditions are disclosed & managed

6.  The original model is returned with the claim

 What’s Not Covered

•  Accidents or biting hard substances (ice, bones, pens)

•  Gum disease, decay, or bone loss

•  Cosmetic concerns after crown is approved and fixed

•  Temporary crowns or provisional prosthesis

•  Shifting of natural teeth , Gums or bite changes over time and problems arising due to it.

•  Warranty claims without the model

 How to Claim Your Warranty

1.  Call or visit SmileSparks for an evaluation

2.  Bring the original model provided at delivery

3.  We will assess and forward the case to the lab

4.  If eligible, a free material replacement will be arranged
